The Resource Exchange (TRE)

The Resource Exchange (TRE) partners with children and adults who have a variety of disabilities, delays, mental health or long-term care needs using a person-centered approach. EARLY CHILDHOOD SERVICES help infants and toddlers with developmental delays, behavioral and/or mental health concerns through a combination of children’s telehealth and in-person parent-child coaching. The Resource Exchange is the Early Intervention provider for infants birth to three years at NO COST to families. NAVIGATION SERVICES assist with the enrollment process into Home and Community Based Service (HCBS) Waiver services and/or other TRE resources. CASE MANAGEMENT serves people of all ages enrolled in a Home and Community Based Service (HCBS) Waiver. FAMILY SUPPORT SERVICES connect families to community supports, education groups, case coordination, and a variety of respite programs, strengthening family bonds. CROSS SYSTEM CARE INTEGRATION brings the healthcare system, social determinants of health, long-term services and supports, and a person's community into common purpose.
