Peak Military Care Network (PMCN)

PMCNThe Peak Military Care Network connects service members, veterans and their families to the highest quality resources provided by our trusted community partners. We help individuals and families connect to any service they might need - financial assistance, education or jobs, behavioral health, child care, family needs or transition support - and we work with them to navigate the many services available in our community to make sure their needs are met. AS an umbrella and training to our partners to ensure they understand military and veteran culture and the needs of our military and veteran community; we also provide information and assistance to individuals and families and case coordination to help ensure access to critical services.

Navigation
We walk beside families, guiding them through complex systems of care, ensuring assistance every step of the way. We refer individuals to our trusted & vetted partners.

Information & Assistance
We provide a central source of information for service members and veterans through call-in, face to face & web-based services. We offer a proactive Network of Care website for secure record storage.

Education & Outreach
We create collaborative relationships with community partners ensuring warm hand offs. We educate & train on cultural competencies for best practices.
